Job summary

Providence at Home with Compassus is hiring a Registered Nurse to lead Hospice care in patients' homes. You will assess needs, plan and coordinate with the IDG, and deliver compassionate nursing services across age groups.

Requires CA RN license, BLS within 30 days, and strong communication. Must be able to drive up to 30 miles and work 1+ days per week; comprehensive benefits available.

Qualifications

• Active RN license in California.
• 1 year hospice/home health or related nursing experience preferred.
• BLS certification within 30 days of hire.
• Valid driver's license required.

Responsibilities

• Complete comprehensive assessments of patients and families.
• Develop, initiate, and revise Plans of Care with the IDG.
• Provide nursing care using the nursing process and administer medications.
• Provide health care instructions to patients and families.
• Coordinate with the Patient Care Manager and IDG for continuity of care.
