Job summary

Chapters Health System is seeking an Admissions Nurse, RN to provide and arrange hospice care in collaboration with the interdisciplinary team. You will perform initial nursing assessments and plan of care, ensuring compassionate service to patients and families.

Qualified candidates hold an RN license, have at least 1 year of nursing experience (hospice or hospital preferred), and are proficient with EMR/EHR systems. A valid driver's license and reliable transportation are required.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Ability to communicate effectively in English (verbal and written); bilingual in Spanish preferred.

Responsibilities

• Complete admissions for all company services including nursing assessments and initial plan of care.
• Provide and explain services to patients and families with empathy and professionalism.
• Collaborate with physicians and care team; obtain orders and initiate care plans.
