Qualifications

• Must have a current RN license in the state.
• Bachelor's degree in nursing from NLN-accredited program acceptable alternative.
• Willing to travel up to 20% of the time, often without notice.

Responsibilities

• Establish and monitor hospice access with community referral sources.
• Promote Bristol Hospice services in the service area.
• Coordinate admissions and communicate with facilities and physicians.
• Provide comprehensive patient assessments and plan of care.
• Review patient information to determine hospice needs and criteria.
• Facilitate hospice referrals and admissions process.
• Serve as liaison between facility, physicians, hospitals, and hospice programs.
