Bristol Hospice Modesto, CA is seeking an Admissions Nurse to establish and maintain relationships with referral sources in the community and coordinate timely admissions.

Qualifications

• Must have an active RN license in California.
• Minimum 1 year of nursing experience within last 3 years or a Baccalaureate in Nursing.
• Ability to travel around 20% and flexible work hours.

Responsibilities

• Relate information to DPCS/ED and Community Education Director to promote hospice access.
• Assess and meet community education needs related to hospice services.
• Maintain perceptions of Bristol Hospice as a high-quality provider.
• Coordinate with referral sources to promote timely admissions.
• Complete hospice consults, admissions, and initial patient/family assessments.
• Communicate with hospice leadership to coordinate care and admissions.
• Provide nursing care utilizing the nursing process and educate patients/families.
