Job summary

Bristol Hospice is seeking an Admissions RN in the San Diego area to establish relationships with referral sources and patients. You will lead hospice admissions, assess patient needs, and coordinate with intake and ED teams to ensure timely access to hospice services.

The ideal candidate holds an active RN license in California, has a BSN or equivalent, and can travel about 20% with flexible hours. Strong communication and organizational skills are essential.

Qualifications

• Must have a current RN license in the state.
• Minimum of one year of nursing experience within the last three years or a BSN from an NLN-accredited program.
• Must be willing to travel 20% of the time, often without notice.
• Flexible in work hours.
• Ability to communicate effectively.
• Experience with hospice philosophy is helpful.

Responsibilities

• Relate information to the DPCS and Community Education Director to promote hospice access.
• Complete hospice consults/evaluations and admissions.
• Coordinate with the intake department, physicians, hospitals, SNFs, and hospice programs for referrals/admissions.
• Provide professional nursing care using the nursing process and perform health assessments.
• May participate in IDT meetings and assist in care planning as needed.
