Job summary

UnitedHealth Group is seeking a Hospice Admissions RN to guide patients and families through the admission process by assessing needs and establishing an individualized plan of care. You will collaborate with physicians, facilities, and the interdisciplinary team to coordinate services and provide skilled interventions.

Responsibilities include coordinating initial care plans, performing on-call duties, documenting progress, and communicating changes to the medical team.

Qualifications

• Current RN license, state(s) you are assigned to work.
• 2+ years of clinical RN experience in acute healthcare.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician
• Functions as a liaison between disciplines and facilities during the admission process
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients’ needs and reports changes in the patient’s condition; obtains physician orders as required
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Performs other duties as assigned
