Job summary

Amedisys, part of the Optum family of businesses, is seeking an RN Case Manager to coordinate compassionate hospice care. You will assess patients, partner with the interdisciplinary team, and develop care plans tailored to each patient’s needs.

Responsibilities include patient assessments, care plan implementation, record-keeping, symptom management, patient/family teaching, and on-call coverage. Requires active RN license and CPR certification with 1+ year of nursing experience.

Qualifications

• Current RN license valid in the state(s) you are assigned to work
• Valid driver's license, reliable transportation and liability insurance
• Current CPR certification
• 1+ years of nursing experience

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness
• Records pain/symptom management changes/outcomes as appropriate
• Teaches patients, families, and caregivers appropriate patient care techniques
• Regularly communicates with clinical manager and care team
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Other duties as assigned
