Job summary

Residential Home Health and Residential Hospice is seeking an Admissions RN to provide end-of-life care to patients in their homes around Scranton, PA. You will conduct admissions assessments, develop plans of care, and educate families, while collaborating with an interdisciplinary team.

The role requires an active RN license in the state, at least two years of RN experience (hospice preferred), and a valid driver’s license with about 20% driving.

Qualifications

• Active RN license in the state you work in.
• Two years of RN experience with hospice experience preferred.
• Valid driver’s license; ability to travel to patient homes in assigned territory ~20% of the day.

Responsibilities

• Deliver hospice admissions care with an interdisciplinary team.
• Perform comprehensive admission assessments and plan of care per physician orders.
• Educate patients and families on the hospice process and care expectations.
• Ensure accurate EMR documentation and timely admissions.
• Travel to patient homes within the Scranton, PA area.
