Residential Home Health and Residential Hospice in Lewisburg, PA is seeking an Admission RN for hospice services in patients' homes. You will conduct comprehensive admissions assessments, coordinate with the interdisciplinary team, and develop individualized plans of care.

The role requires an active RN license, at least two years of RN experience with hospice preferred, and a valid driver's license for ~20% travel. A $10,000 first-year bonus is available.

Qualificações

• Active RN license in the state you work.
• Two years of RN experience with hospice experience preferred.
• Valid driver's license; ability to travel ~20% of time.

Responsabilidades

• Deliver compassionate, patient-centered hospice admissions assessments and plans of care.
• Educate patients and families about hospice process and care transition.
• Document accurately in EMR and coordinate with interdisciplinary team.
