Job summary

Graham Healthcare Group is seeking an Admission Registered Nurse (RN) for Hospice to provide end-of-life care to patients in their homes around Lewisburg, PA. The role requires compassionate, patient-centered care and collaboration with an interdisciplinary team.

You will conduct admissions assessments, educate families on hospice, document in EMR, and travel to patients' homes within the assigned territory. A valid RN license and driving for 20% travel are required.

Qualifications

• Active RN license in the state you work
• Two years RN experience with hospice preferred
• A valid driver's license and ability to travel ~20% of the workday
• Authorized to work in the United States

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team
• Perform comprehensive hospice admission assessments and develop care plans per physician orders
• Educate families on the hospice admission process and philosophy, ensuring informed decisions
• Apply clinical judgment to facilitate timely admissions and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but not alone and travel to patient homes within territory
