Job summary

Residential Home Health and Hospice seeks an Admissions RN to perform hospice admissions in Scranton, PA. This role requires assessing patients, coordinating care plans, and educating families about hospice services. A valid RN license and driving to patient homes are essential.

Qualifications

• Active RN license in the state you work
• Two years of RN experience with hospice/home health preferred
• Demonstrated ability to perform comprehensive hospice admissions and education

Responsibilities

• Deliver compassionate, patient-centered hospice admissions in patients’ homes
• Conduct comprehensive admission assessments and plan care per physician orders
• Educate families on hospice philosophy and transition process
• Apply clinical judgment to ensure timely admissions and accurate EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but collaborate with a multidisciplinary team
• Travel within assigned territory to patient homes
