Residential Home Health and Hospice is seeking an Admissions RN for hospice services in Mechanicsburg, PA, traveling to patient homes. You will conduct comprehensive hospice admissions, assess needs, reconcile meds, and develop plans of care with physicians.

A valid RN license and at least two years of RN experience are required, plus a driver’s license for 20% travel. We offer a $10,000 first-year bonus and a supportive team environment.

Qualificações

• Active RN license in the state you work
• Two years of RN experience and the clinical proficiency for hospice care
• A valid driver’s license and ability to travel ~20% of workday

Responsabilidades

• Deliver patient-centered hospice admissions assessments and develop plans of care
• Educate patients and families about hospice processes and expectations
• Document admissions and coordinate with interdisciplinary team
• Travel to patient homes within assigned territory
