Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in Northbrook, Illinois. You will provide end‑of‑life care in patients’ homes, performing admissions assessments, care planning, and family education as part of an interdisciplinary team.

This full‑time role requires an active RN license in IL, about 2 years of RN experience (hospice preferred), and a valid driver’s license with ~20% travel.

Qualifications

• Active Registered Nurse license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to travel ~20% of your workday driving to patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders
• Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care
• Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but never alone
• Travel to patient homes within assigned territory
