Graham Healthcare Group is seeking an Admission Registered Nurse (RN) to provide end-of-life hospice care in patients' homes in the Northbrook, IL area. This role offers a $9,000 first-year bonus for achieving key milestones.

You will perform hospice admission assessments, educate families, and coordinate with an interdisciplinary team to ensure compassionate, high-quality care. A valid IL RN license and driving will be required.

Qualifikationen

• Two years of RN experience; hospice experience preferred.
• Active RN license in IL.
• Ability to drive to patient homes and work in home-based hospice settings.

Aufgaben

• Deliver patient-centered hospice care in patients' homes.
• Conduct admission assessments, including medication reconciliation and development of individualized plans of care.
• Educate patients and families about hospice processes and expectations.
• Apply clinical judgment to ensure timely admissions and compliant EMR documentation.
• Coordinate evaluations with other disciplines as needed.
• Work independently while collaborating with the interdisciplinary team.
• Travel to patient homes within assigned territory.
