Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in Palatine, Illinois. You will provide end‑of‑life care in patients' homes within the Wheeling, IL area, collaborating with an interdisciplinary team and delivering compassionate, patient‑centered care.

This full‑time role requires an active RN license, at least two years of RN experience (hospice preferred), and a valid driver's license for ~20% travel. A first‑year $10,000 success bonus is offered.

Qualifications

• Active RN license in the state you work
• Two years of RN experience; Hospice experience preferred
• Must have valid driver’s license and be able to drive ~20% of workday

Responsibilities

• Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning
• Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
