Qualifications

• Active Registered Nurse license in the state you work (IL).
• Two years of RN experience with hospice care preferred.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Conduct comprehensive hospice admission assessments, including clinical evaluations and care plans.
• Educate patients and families through the hospice admission process, setting expectations.
