Job summary

Graham Healthcare Group, a leading provider of residential home health and hospice services in Illinois, seeks a Hospice RN to deliver compassionate end-of-life care in patients’ homes in the Deerfield area. This ongoing, full-time position offers autonomy, strong teamwork, and opportunities for growth.

The ideal candidate holds an active Illinois RN license, has hospice experience, and can drive to patient locations about 20% of the time.

Qualifications

• Active Registered Nurse (RN) license in the state of practice.
• Two years RN experience; hospice experience preferred.
• Valid driver's license and ability to drive ~20% of time.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes with an interdisciplinary team.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families to ensure they feel informed, prepared, and cared for.
• Think critically, ensure compliant EMR documentation and maintain safety practices.
• Work independently, but collaborate with the team and travel to patient homes within territory.
• Travel to patient homes within assigned territory.
