Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing role in Hinsdale, Illinois. This diverse, home-based position supports patients and families at the end of life.

You’ll conduct assessments, manage symptoms, coordinate with an interdisciplinary team, and document in EMR. Travel about 20% of workdays to patient homes within your territory.

Qualifications

• Active RN license in Illinois.
• Two years of RN experience, hospice experience preferred.
• Must have valid driver's license and ability to travel up to 20% of work time.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Perform comprehensive patient assessments and symptom/pain management; participate in end-of-life care planning.
• Educate and support patients and families to help them feel informed and cared for throughout the journey.
