Job summary

Graham Healthcare Group is seeking an Admission RN for Hospice to deliver compassionate in-home end-of-life care across the Lisle, IL area. This role requires a valid RN license and a couple of years of experience, with travel to patient homes and a strong emphasis on patient and family education during the admission process.

The position includes a dedicated field education team, mentorship, and opportunities for professional growth within a nationally recognized home health and hospice

Qualifications

• Active Registered Nurse (RN) license in the state you work.
• Two years of RN experience; hospice experience preferred.
• A valid driver’s license and ability to drive ~20% of work time.

Responsibilities

• Deliver patient-centered hospice care in patients' homes as part of an interdisciplinary team.
• Conduct comprehensive hospice admissions assessments and develop care plans.
• Educate families about hospice processes and expectations during transition to end-of-life care.
• Apply clinical judgment to ensure timely admissions, accurate documentation, and safety.
• Coordinate with other disciplines when indicated and travel to patient locations.
