Job summary

Graham Healthcare Group in Montandon, PA is seeking a dedicated Registered Nurse (RN) for Hospice admissions. You will conduct comprehensive hospice admission assessments, educate patients and families, collaborate with an interdisciplinary team, and travel to homes within your territory.

A valid RN license and at least two years of RN experience are required; a first-year $10,000 success bonus applies in addition to standard compensation.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes with an interdisciplinary team.
• Conduct comprehensive hospice admission assessments and develop care plans.
• Educate patients and families about the hospice process and transition to end-of-life care.
• Apply clinical judgment to facilitate timely admissions and accurate EMR documentation.
• Initiate evaluations to other disciplines when indicated.
• Travel to patient homes within the assigned territory.
