Job summary

Graham Healthcare Group in Milton, PA is seeking a Registered Nurse (RN) Hospice to deliver compassionate, patient-centered end-of-life care in patients' homes within your territory.

You’ll conduct hospice admissions assessments, develop plans of care, educate families, and ensure accurate EMR documentation, traveling up to 20% of your time. Eligible for a $10,000 first-year success bonus and a full benefits package.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Must have a valid driver's license and be able to drive to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Conduct comprehensive hospice admission assessments and develop care plans.
• Educate patients and families about the hospice process and expectations.
• Ensure accurate EMR documentation and appropriate referrals when needed.
• Coordinate with interdisciplinary team and other disciplines as indicated.
• Travel to patient homes within the assigned territory and work independently.
