Job summary

Graham Healthcare Group in Elizabeth, PA is seeking a Hospice Registered Nurse (RN) to deliver compassionate end‑of‑life care in patients' homes within the assigned territory.

This staff role requires 40 hours per week with an 8-hour shift, travel to patient homes, and a comprehensive benefits package. A $10,000 first-year success bonus is included to recognize milestone achievement.

Active RN license, two years of RN experience (hospice preferred), and a valid driver’s license are required.

Qualifications

• Active RN license in the state you work
• Two years of RN experience and hospice experience preferred
• Valid driver’s license with ability to travel ~20% of the time

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning
• Educate and support patients and families through the end-of-life journey
• Think critically, solve problems in real time, and ensure compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
