Graham Healthcare Group seeks an experienced Registered Nurse (RN) to provide compassionate hospice care in patients' homes around Danville, PA. This role includes travel to patient locations and a first-year success bonus up to $10,000.

Enjoy autonomy, meaningful patient relationships, and career growth within a nationally recognized home health and hospice provider. A valid RN license, two years of RN experience, and a driver’s license are required.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of day driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
