You will deliver interdisciplinary, patient-centered care, perform assessments and symptom management, educate families, and maintain compliant EMR documentation. This field-based role requires a valid driver's license and about 20% travel. Eligible for a $10,000 first-year success bonus.

Qualifications

• Active RN license in the state you work.
• Minimum 2 years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive ~20% of workday.

Responsibilities

• Deliver patient-centered hospice care with interdisciplinary team.
• Perform comprehensive patient assessments and pain management.
• Educate families through the end-of-life journey.
• Document thoroughly in EMR.
• Work independently but collaborate as needed.
• Travel to patient homes within territory.
