Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) for Hospice to provide end-of-life care in patients' homes in the Westchester, IL area. The role requires 40 hours per week, 8-hour shifts, and staff employment status, with travel to patient locations within the territory.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; Hospice experience preferred.
• Must have valid driver’s license and ability to travel ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
• Think critically, ensure EMR documentation is thorough and compliant.
• Work independently, but collaborate with team and travel to patient homes.
