Graham Healthcare Group is seeking an Admission Registered Nurse (RN) for Hospice in Lisle, Illinois. This role provides end-of-life care to patients in their homes and offers a $10,000 first year success bonus to reward key achievements during your first year!

Qualifications

• Active Registered Nurse, RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Conduct comprehensive hospice admission assessments and develop plans of care.
• Educate patients and families about the hospice admission process and care expectations.
• Apply clinical judgment to facilitate timely admissions and accurate EMR documentation.
• Initiate evaluations with other disciplines when indicated.
• Work independently, but collaborate with team; travel to patient homes.
