Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice in Westmont, Illinois. This full-time role offers 40 hours per week with 8-hour shifts located in home settings. A $10,000 first-year success bonus is available for milestone achievements.

You will provide end-of-life care, conduct symptom management, and educate families within an interdisciplinary team. Travel to patient homes within the assigned territory is required.

Qualifications

• Active RN license in the state you work in.
• At least 2 years of RN experience, hospice preferred.
• Valid driver’s license and ability to drive ~20% of work time.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Perform comprehensive patient assessments and pain/symptom management.
• Educate and support patients and families through end-of-life care.
