Job summary

Eligible for a $10,000 first-year bonus; strong autonomy, growth opportunities, and support from a dedicated field education team. Travel required to patient homes; IL RN license and 2 years' RN experience preferred.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver's license and ability to travel ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Perform assessments, symptom and pain management, and end-of-life care planning.
• Educate families and provide support throughout the journey.
• Maintain accurate EMR documentation and adhere to compliance.
• Work independently within an interdisciplinary team and travel to patient homes.
