Hospice Home Care in Hot Springs, AR is seeking a compassionate Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, and facilities, explain services, collect medical history, and coordinate the start of hospice services with the care team.

We offer flexible scheduling, 1:1 patient care, team support, and growth opportunities within our Adoration Health affiliation. This role emphasizes patient comfort and compassionate care during a critical life phase.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or related settings to be considered.
• Must be a licensed driver with insured automobile in good working order.

Responsibilities

• Visit patients in homes, hospitals, nursing facilities, etc. who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
