Hospice Home Care in Hot Springs, AR, seeks a Hospice Admissions RN to join our team. Visit patients in homes and facilities to assess needs, explain hospice services, and coordinate start of care with the interdisciplinary team.

Must have RN licensure, hospice experience preferred, and a valid driver’s license with insured vehicle. We offer comprehensive benefits and opportunities for growth within a supportive setting.

Formación

• Graduate of an accredited nursing program with state licensure.
• Minimum 1 year hospice nursing experience; 3+ years in related settings may count.
• Valid driver's license and insured vehicle.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
