Hospice Home Care in Little Rock, AR is seeking an experienced Hospice Admissions RN to join our team. You will visit patients in homes and facilities, explain hospice options, assess needs, and coordinate service starts with an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, ICU, or ER experience could be considered.
• Must be a licensed driver with insured automobile in good working order.

Responsibilities

• Visit patients referred for hospice care in homes, hospitals, nursing facilities, etc.
• Explain hospice services, discuss goals and care with patient and families, obtain consents and documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice per Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team and maintain timely documentation.
