Job summary

Hospice Home Care in Hot Springs, AR, is seeking a dedicated Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, and facilities to assess eligibility for hospice services and coordinate initiation of care.

The role emphasizes compassionate, patient-centered assessments, collaboration with the interdisciplinary team, and thorough documentation. Schedule is M-F, 9 a.m.–6 p.m., with opportunities for growth and stability within a supportive environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care.
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
