Qualifications

• Graduate of an accredited nursing program with state licensure.
• Minimum 1 year hospice nursing experience; 3+ years in related settings may count.
• Valid driver's license and insured vehicle.

Responsibilities

• Visit patients in homes, hospitals, or facilities to assess and initiate hospice care.
• Explain services, goals, collect consents, and review medical history.
• Coordinate start of services and collaborate with the care team.
• Maintain timely documentation and charting.
