Job summary

Queen City Hospice in Cincinnati, OH is seeking an experienced Hospice Admissions RN to guide patients and families at the start of their hospice journey. This role offers a compassionate, independent practice with a strong emphasis on quality of life and patient-centered care.

You'll conduct pre-admission evaluations, educate families about services, obtain consent, and develop individualized care plans, all while enjoying a supportive team, manageable caseloads, and a scheduling model that

Qualifications

• Graduate of an accredited RN program.
• Active Registered Nurse license in the state of practice.
• Current CPR certification.
• Hospice experience (preferred, not required).
• Strong clinical judgment and ability to work independently.
• Compassionate communication skills with patients, families, and care teams.
• Understanding of quality assurance and patient care standards.

Responsibilities

• Conduct pre-admission evaluations to determine eligibility and appropriateness of care.
• Educate patients and families about hospice services with compassion and clarity.
• Obtain informed consent and initiate the admission process.
• Perform comprehensive nursing assessments and develop individualized care plans.
• Deliver interventions that maximize comfort, dignity, and quality of life.
