Job summary

Clear Concepts, Inc. is seeking a dedicated Hospice Admissions RN in the Kansas City metro area (MO). The role covers a 50-mile radius from the office with a Tue-Sat 10am-7pm schedule, coordinating new patient admissions and conducting comprehensive assessments.

You will educate patients and families while partnering with the care team for a smooth transition into hospice services. Required are an MO RN license, CPR, two years RN experience in a clinical setting, and one year of hospice

Qualifications

• Active Missouri RN license required.
• Current CPR certification required.
• Two years RN experience in a clinical setting.
• One year hospice experience preferred.
• Driver's license and reliable transportation required.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills
