Job summary

Ridgemont Resources, Inc. in the Kansas City metro area (east) seeks a Hospice Admissions RN to coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and collaborate with the interdisciplinary care team to ensure a smooth transition into hospice services.

The role is full-time with a 50-mile radius from the office and a Tue-Sat, 10am-7pm schedule. Missouri RN license, CPR certification, and hospice experience are required.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities
