Job summary

Software Placement Group seeks an experienced RN Case Manager for full-time, field-based hospice care around the Kansas City metro area. You will coordinate comprehensive patient care, develop plans with the interdisciplinary team, and ensure timely documentation.

The role emphasizes patient/family education, symptom management, and strong collaboration with physicians and leadership. MO licensure, RN experience, and CPR are required.

Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Responsibilities

Overview

A respected hospice care provider is seeking a full-time RN Case Manager to coordinate and oversee patient care in the Kansas City metro area. This role is central to delivering compassionate, individualized care to hospice patients and their families through clinical expertise and close interdisciplinary collaboration.
