Interim HealthCare Hospice is hiring a Hospice Admissions RN to provide comfort, strength and guidance to patients and families at life’s final chapter. Conduct initial home or facility assessments, start the plan of care, and coordinate admissions with the IDT and physicians.

The role requires an Indiana RN license and at least 1 year of nursing experience, preferably in hospice or home health care, with CPR certification.

• CPR certification is required.

Responsabilidades

• Conduct timely initial hospice assessments in patients' homes or facilities, documenting physical, psychosocial, and medication histories to determine eligibility and care needs.
• Initiate the interdisciplinary plan of care by writing nursing diagnoses, setting palliative goals, and obtaining physician orders and consent forms at admission.
• Coordinate referrals, verify insurance, and ensure smooth admissions as the clinical liaison among patients, families, physicians, and IDT.
• Provide start-of-care nursing interventions, symptom management teaching, and caregiver education to optimize comfort and safety.
• Document all assessments, care plans, and communications in the EMR in compliance with Medicare/state regulations.
• Educate family on prognosis, changes in care plan, medications and hospice goals.
• Take a holistic approach engaging the patient’s mind, body, spirit and family.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

• Conducts timely initial hospice assessments in patients’ homes or facilities, completing comprehensive physical, psychosocial, and medication histories to determine eligibility and care needs.
• Initiates the interdisciplinary plan of care by writing nursing diagnoses, setting palliative goals, and obtaining physician orders and hospice election/consent forms at admission.
• Serves as the clinical liaison among patients, families, physicians, intake staff, and the IDT to coordinate referrals, verify insurance, and ensure smooth, compliant admissions.
• Provides skilled start-of-care nursing interventions, symptom management teaching, and caregiver education to optimize comfort and safety from day one of hospice care.
• Documents all admission assessments, care plans, and communications accurately and promptly in the EMR in accordance with Medicare, state regulations, and agency timeliness policies.
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

A few must-haves for Hospice Registered Nurses:

• Graduate of an accredited nursing program and active RN license in Indiana
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
