Job summary

Interim HealthCare Hospice in Fort Wayne, IN is hiring a Hospice Admissions RN to provide compassionate admissions—assessing patients in homes or facilities, coordinating referrals, verifying insurance, and initiating the interdisciplinary care plan. You’ll educate families and ensure compliant admissions from day one.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Indiana.
• Minimum 1 year nursing experience, ideally in hospice or home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.

Responsibilities

• Conducts timely initial hospice assessments in patients' homes or facilities.
• Initiates interdisciplinary plan of care by writing nursing diagnoses and obtaining orders.
• Coordinates referrals, verifies insurance, and ensures compliant admissions.
• Provides start-of-care nursing interventions and caregiver education.
• Documents assessments and care plans in EMR per Medicare/state policies.
• Educates family on prognosis and care plan changes.
• Approaches care holistically engaging patient, family, mind, body, and spirit.
