Job summary

Interim HealthCare Hospice seeks an experienced RN to provide compassionate, home-based end-of-life care in Hudson, Wisconsin. You will work with an interdisciplinary team to deliver high-quality comfort-focused care.

The role requires Wisconsin RN licensure, CPR certification, and at least 1 year of nursing experience (hospice or home health preferred). Competitive salary and benefits are offered with mileage reimbursement and growth opportunities.

Qualifications

• Graduate of an accredited nursing program with an active Wisconsin RN license.
• Minimum of 1 year nursing experience; hospice or home health preferred.
• CPR certification is required; knowledge of home health regulations is essential.
• Clinical proficiency in nursing and strong communication and compassion.

Responsibilities

• Provide home-based care to patients with six months or less to live, focusing on quality of life.
• Collaborate with an interdisciplinary team including physician, LPN, CNA, HHA, social worker, chaplain and volunteers.
• Assess patients, monitor vitals, evaluate breathing, manage pain, and administer prescribed meds and treatments.
• Ensure access to medical equipment, oxygen, medications and supplies as needed.
• Educate family on prognosis, care plan changes and hospice goals.
• Approach care holistically, engaging patient, family, mind and spirit.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
