Job summary

Interim HealthCare Hospice is seeking a Hospice Registered Nurse (RN) on an on-call cycle of 7 on, 7 off in Wyoming, MI. This full-time role involves leading home-based palliative care with an interdisciplinary team to ensure comfort and quality of life for patients and families.

The position offers a salaried 80k-95k annual range with weekly bonus opportunities, plus health/dental/vision/life insurance, 401(k)/Roth, paid vacation, mileage reimbursement, and ESOP.

Qualifications

• RN with active, unrestricted license in the State of Michigan (required)
• At least 1 year of RN experience (required)
• 1 year of home health experience preferred
• Current CPR Certification (required)
• Reliable transportation with valid driver's license and current insurance (required)

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer medications and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and supplies
• Educate family on prognosis, changes in care plan, and hospice goals
• Take a holistic approach to care engaging patient’s mind, body, spirit, and family

Looking for a full time nurse for this position! 7 days on (4:30PM-8AM M-F, 24/7 Saturday and Sunday) and then off the next 7 days.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

RN Requirements

• RN with active, unrestricted license in the State of Michigan
• At least one (1) year of relevant experience as an RN (required)
• One (1) year of management experience (preferred)
• One (1) year of home healthexperience (preferred)
• Current CPR Certification
• Community health and a wide range of Medical-Surgical experience (preferred)
• Reliable transportation with a valid state driver's license and current car insurance
