Reserve Care is seeking a Clinical Admissions Nurse to assess and admit patients for hospice services in home or facility settings. The role supports the hospice philosophy and collaborates with the Referral Department to coordinate care.

We require an Ohio RN license with BSN or higher and at least one year of nursing experience in home care, hospice, or oncology. This is an exempt, full‑time position with benefits and weekend scheduling.

Qualifications

• Ohio RN license and BSN preferred.
• Experience in hospice, home care, or oncology nursing preferred.
• Strong communication and teamwork are essential.

Responsabilités

• Perform duties in accordance with the Nurse Practice Act and professional nursing principles.
• Provide information, assessment, and admission services to referred patients in home, facility, or health care setting.
• Coordinate with Referral Department and physicians to ensure appropriateness and necessary certifications.
• Maintain documentation accurately and timely in the EMR system.
• Collaborate with the IDT to coordinate care and meet patient needs efficiently.
