Qualifications

• Current and active California RN license.
• Minimum one year acute care experience required.
• One year recent home health experience preferred.
• Current CPR certification.
• Licensed driver with insured automobile.
• Management experience not required.
• Excellent observation, verbal and written communication skills, problem solving skills.

Responsibilities

• Conduct continuous review of patient caseload, ensuring appropriate utilization of services and discharge planning.
• Perform initial patient/family assessment and establish level of care.
• Develop and revise plan of care based on nursing diagnosis and patient needs.
• Administer medications and treatments as prescribed. Reassess patient needs regularly.
• Counsel patient and family, provide health education, and ensure safe home environment.
• Coordinate with physician and care team; participate in on-call duties and QA activities.
