Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the state.
• Maintain CPR certification where required.
• Must have reliable transportation and a valid driver's license.
• Demonstrate strong verbal and written communication and organization skills.

Responsibilities

• Give hands-on end-of-life care following the care plan.
• Assess patients and follow care plans with state nursing rules.
• Keep clear, timely records of care provided.
• Educate patients and families about illness and care plan.
• Coordinate care with hospice team to ensure smooth service.
