Hospice Home Care in Little Rock, AR is seeking a dedicated Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, or facilities, assess eligibility, and explain hospice services to patients and families.

A current AR RN license and valid driver's license are required. We offer flexible scheduling, strong team support, and opportunities for professional growth within a growing company.

Competenze

• Must hold a current Registered Nurse license in Arkansas.
• Minimum of 1 year hospice nursing experience; 3+ years in related settings considered.
• Must be a licensed driver with reliable vehicle.

Mansioni

• Visit patients referred for hospice care at home, hospital, or facility.
• Explain hospice services, discuss goals with patients and families, obtain consents and required documents.
• Collect medical history, medications, and symptom data; perform assessments for Medicare/insurance eligibility.
• Coordinate initiation of services (nursing, aides, equipment, medications) and collaborate with the care team.
• Maintain accurate, timely documentation of all patient interactions.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
