Job summary

Advocate Health is seeking a dedicated Hospice Intake Coordinator to manage referrals, coordinate admission timelines, and ensure compliance with Medicare and managed care guidelines. The role requires strong communication and a collaborative approach with referral sources.

The position supports the hospice administration team, offers full-time hours, and includes benefits. On-call rotation may be required; there is a commitment to compliant, quality patient care across the organization.

Qualifications

• HS Diploma or equivalent; 3 years hospice intake experience preferred.
• Very good communication skills and customer service orientation.
• Knowledge of Medicare, Medicaid, HMO procedures and managed care guidelines.

Responsibilities

• Coordinate all hospice referrals and ensure compliance with state and federal regulations; act as the internal point person for new referrals.
• Provide liaison with referral sources and admission teams to coordinate scheduling and documentation.
• Respond to inquiries about Hospice care and maintain up-to-date knowledge of Medicare/Medicaid procedures.
• Identify risk management situations and report to supervisor.
• Cross-reference patient data in the EMR and coordinate necessary admission documents.
