Job summary

Trinity Health At Home Hospice in Columbus, OH, is seeking an experienced Hospice Admissions Manager to lead the admissions team from referral to admission, ensuring seamless transitions of care and alignment with the organization's mission.

This leadership role supervises admission RNs and coordinators, coordinates referrals and admissions by geography, tracks performance metrics, and partners with physicians and site leadership to drive quality and patient-centered care.

Qualifications

• Current licensure in the state(s) of practice appropriate to discipline.
• Minimum one year of supervisory or leadership experience.
• Valid driver's license, reliable transportation, and state-required automobile liability insurance.
• Ability to travel throughout the service area as needed.
• Two or more years in hospice, home health, oncology, or related patient care settings preferred.

Responsibilities

• Provide direct supervision and support to Admission RNs, admissions coordinators, and liaisons.
• Oversee referral and admission processes from initial referral through patient admission.
• Monitor referrals, ensure timely follow-up and conversion to appropriate levels of care.
• Coordinate smooth transitions from Admissions to Patient Care teams.
• Ensure compliance with regulatory requirements and quality improvement initiatives.
• Educate and partner with community providers to support seamless transitions of care.
