Job summary

Capstone Hospice is seeking a dedicated and empathetic Clinical Liaison (RN) to join our admissions team in the Metro Atlanta area. You will facilitate hospital-to-home hospice transitions, coordinate with healthcare providers, patients, and families to deliver comprehensive end-of-life care with dignity and comfort.

The ideal candidate is an RN with hospice experience, strong interpersonal skills, and proficiency in EMR/EHR systems.

Qualifications

Nursing degree with current active Georgia nursing license as a Registered Nurse (RN).
Minimum of 3 years’ clinical experience, with 1–2 years in hospice or palliative care.
CPR certification.
Experience & proficiency in EMR/EHR systems.
Direct hospice care experience as Hospice Admissions, Hospice RN Case Manager, or Hospice Visit Nurse.

Responsibilities

Coordinate patient transitions from hospital to home hospice care, collaborating with hospital and hospice teams.
Serve as primary liaison between hospitals, patients, and families to communicate care plans and goals.
Provide emotional and informational support to patients and families throughout the hospice process.
Ensure regulatory compliance and monitor patient outcomes and satisfaction.
