Trinity Health At Home Hospice is seeking an experienced and mission-driven Hospice Admissions Manager to lead and support our admissions team in delivering exceptional end-of-life care. This leadership role is responsible for overseeing the admissions process from referral through admission, ensuring seamless transitions of care while supporting the interdisciplinary team and promoting Trinity Health's mission, values, and commitment to excellence.

The Hospice Admissions Manager serves as a key clinical and operational leader, providing guidance, education, and oversight to admission nurses, admissions coordinators, liaisons, and other direct patient care team members.

Position Summary

The Hospice Admissions Manager provides leadership, direction, and oversight for hospice admission activities to ensure timely, compliant, and patient-centered care. This role supervises admission staff, coordinates referral and admission processes, collaborates with physicians and operational leaders, and champions quality improvement initiatives that enhance the patient and family experience.

Key Responsibilities

Leadership & Team Management

  • Provide direct supervision and support to Admission Registered Nurses, admissions coordinators, and liaisons.
  • Ensure appropriate staffing levels, scheduling, orientation, and ongoing staff development.
  • Participate in hiring, performance evaluations, coaching, and employee engagement activities.
  • Lead and facilitate admissions team meetings and interdisciplinary discussions as needed.
  • Collaborate with site leadership to support education, compliance, and quality initiatives.

Admissions Operations

  • Oversee the hospice referral and admission process from initial referral through patient admission.
  • Ensure referrals and admissions are coordinated efficiently and assigned appropriately based on geography, staffing, and patient needs.
  • Monitor pending referrals and ensure timely follow-up and conversion to appropriate levels of care.
  • Coordinate smooth transitions from Admissions to Patient Care teams.
  • Prepare end-of-day reports and communicate patient status updates to on-call teams.
  • Oversee General Inpatient (GIP) contract bed admissions and non-formulary treatment approvals when applicable.

Clinical Excellence & Patient Care

  • Ensure hospice eligibility determinations and level-of-care decisions comply with regulatory requirements and organizational standards.
  • Collaborate closely with the Hospice Medical Director, attending physicians, and clinical leadership.
  • Support symptom management planning and development of appropriate initial plans of care.
  • Ensure all required assessments and documentation are completed accurately and within required timeframes.
  • Promote best practices in patient-centered hospice care.

Quality & Compliance

  • Monitor departmental productivity, referral conversion rates, and operational outcomes.
  • Participate in quality assurance and performance improvement (QAPI) activities.
  • Conduct root cause analyses for referral or admission concerns and implement corrective action plans.
  • Perform chart audits and documentation reviews to ensure compliance with federal, state, and organizational standards.
  • Assist with monitoring corporate compliance initiatives and quality metrics.

Community & Relationship Management

  • Provide education and in-service training to community partners, healthcare providers, facilities, and referral sources.
  • Foster strong relationships with hospitals, physicians, and healthcare organizations to support seamless transitions of care.
  • Serve as a resource for patients, families, physicians, and community partners regarding hospice services.
Qualifications
  • Demonstrated commitment to Trinity Health's Mission, Values, Code of Ethics, and Compliance Program.
  • Current licensure in the state(s) of practice appropriate to discipline.
  • Minimum one (1) year of supervisory or leadership experience.
  • Valid driver's license, reliable transportation, and state-required automobile liability insurance.
  • Ability to travel throughout the service area as needed.
Preferred
  • Registered Nurse (RN) license.
  • Certified Hospice and Palliative Nurse (CHPN) certification.
  • Current CPR certification.
  • Two (2) or more years of experience in hospice, home health, oncology, medical-surgical nursing, geriatrics, pediatrics, or related patient care settings.
  • Previous hospice admissions or care transition experience.

  • Strong clinical assessment and hospice eligibility determination skills.
  • Excellent leadership, coaching, and team development abilities.
  • Strong communication and relationship-building skills.
  • Knowledge of hospice regulations, compliance standards, and quality improvement practices.

  • Proficiency in documentation and electronic medical record systems.
Physical Requirements
  • Ability to stand, walk, and move throughout the workday.
  • Ability to lift up to 35 pounds independently.
  • Ability to bend, stoop, twist, and assist with patient positioning when necessary.
  • Ability to perform detailed work despite frequent interruptions and changing priorities.
  • Ability to travel to various work locations as required.
