Greater Horizon Healthcare in Boston, MA is seeking two Licensed Practical Nurses (LPNs) for weekday and occasional weekend rotations. This part-time role offers a few days per week with a hybrid work location and a focus on personalized patient care.

Applicants must hold a Massachusetts Nursing License and LPN certification, CPR, TB test, and a background check. English proficiency and the ability to work independently are essential, with attention to safety and compassionate care.

Qualifications

Massachusetts Nursing License is required.
LPN certification is required.
CPR Certification required.
TB skin test and Physical updated.
Background check required.
Driver's License/State ID required.
Must be able to work independently with minimal supervision.
English language proficiency mandatory.

Responsibilities

Demonstrates knowledge of basic health care provider skills.
Follows standard precautions and other infection control protocols as needed.
Provides 1:1 nursing care to residents who may require restraints or special observation.
Understands residents' rights and responsibilities.
Provides comprehensive physical care to residents.
Motivates residents towards optimal level of functioning.
Accurately records care delivery and observations using established documentation guidelines.
Provide a range of patient/resident care and contribute to the nursing care plan.
